- We will evaluate, plan, and coordinate the care needed with you. Our RN will meet with you and your loved one at home or on-site to conduct an initial consultation and comprehensive assessment to encompass client's health condition, safety, medical/non-medical goals, and family concerns. Our RN will create a 'Life at Home' plan integrating the data, recommendations from health care providers, and family.
- The goal of the individualized 'Life at Home' plan is to facilitate well-being, crisis prevention, independence, keeping the client safely at home as long as possible, and ensure continuity of care. We will strive to go above and beyond our motto "Our Mission is You and Our Vision is Compassion".
